Also, do you have to present documentation of this when taking your cdl permit test?

Yes, You can have it done by your family doctor but, you will have another one during school and before you are hired by a company. After that you can get the physical done by and licensed doctor
Yes, In most states you do need the DOT physical. Here in PA you do not need physical before you get your permit. You will need one before you go to school though

Sorry but you are only partially correct.
Yes you can have a DOT physical done by your family doctor however:
Many companies will not accept these physicals and require that you take one during orientation using their clinic/doctor.
There are some companies around though that will accept a current, valid physical as long as it recent.
As far as schools go. Pretty much the same thing.
Bottom line. Just depends on company/facility policies.
As far as having to have a physical to take the written tests. No you do not need one.
Final Note: Not all family practicianers will perform DOT physicals.

Some companies dont want to go through the hassle of doing a DOT physicals. Easier I think
