Question:
I spent 5 years "not driving",when I got back into it 4 months ago(local nursery doing regional stuff) I went to the clinic they sent me to for my phys. Got my card..it was not the old familiar card, it was a piece of paper without all the old familiar info!?!? I asked the doc about it and he said they changed it. Anyone know for sure? What all do I need to carry to be legal? I've forgotten more than I ever really knew..LOL.

Sounds like you have the long form. It should have the same info and more that is on the card. I have both. I keep the long one in my brief case and the card in my wallet.
I also heard but don't know for sure that the long form is required. I have shown my card several times and never been asked for anything else. It is what I've heard though and might be worth looking into. But, you are legal with what you have.
