Endometrioma - chocolate cyst
Question:
has anyone had an endometrioma or a chocolate cyst? If so, what were
your symptoms and what was the treatment? Mine is 5cm so
far.....started BC pills this week....

Hey mommy0moo, how are you doing? The goal of directed medical
treatment is to achieve an anovulatory state. This can also be
accomplished with progestational agents (ie, medroxyprogesterone),
danazol, gestrinone, or gonadotropin-releasing hormone agonists
(GnRH), as well as other less well-known agents if oral contraceptives
and NSAIDs are ineffective in controlling symptoms. Endometrosis
and other gynecologic disorders hardly ever manifest themselves by
back pain. Menstrual pain is poorly localized, tends to radiare down
the legs and is of a crampy nature. Endometrosis or carcinoma of
the uterus (body or cervix) may invade the uterosacral ligaments.
Malposition of the uterus may pull on them. Pain is localized
centrally in the sacrum below the lumbosacral joint, but may be more
on one side. Pain begins during the premenstrual phase, often
continues until it merges with menstrual pain. Malposition of the
uterus (retroversion, descensus, and prolapse) leads to sacral pain -
especially after the patient has been standing for serveral hours.
Carcinomatous pain implication of nerves plexuses is continuous and
becomes more severe as it progresses. Endometrosis tends to be more
intense at night. Pain in the back may be consequense of arthritis of
the cervical of thoracic spine. GnRH can be combined with estrogen
and progestogen (add-back therapy) without loss of efficacy but with
fewer hypoestrogenic symptoms. No evidence exists that any one of
these agents is most effective in controlling pain; moreover, there is
no strong evidence that they improve fertility. Conservative
surgery can be performed to preserve fertility in young patients.
Definitive surgery is a hysterectomy and bilateral oophorectomy.

Thank you so much for your reply. This has been a horrible couple of
weeks - this pain is worse than labor - seems as if there is no end!!
I go in for another U/S on Thursday - the cyst potentially ruptured
over a week ago and have been bleeding for 9 days straight. I am
exhausted, dizzy and weak. Nightime is definitely worse - cannot
sleep for the excruciating pain and swelling.... I am ready to
'leap up on the table' hysterectomy sounds really nice right about
now. I'm only 31, but have not been able to conceive for 3 years -
PCOS, Insulin resistant, 1 sucessful pregnancy 5 years ago, 1 m/c 3
years ago in May. Nurse told me she thinks that my cyst is leaking
so they will look to see if that's the case. I hope if anyone else
has had this type of cyst that they can share their experience and
treatments. I will search eMedicine now - thank you so much for the
advice - Iwill also ask about the meds you suggested. Thank you

Hi mommy0moo! I found out this past Monday that the left ovarian
cyst I have been taking BC pills for the last 6 weeks is most likely
an endometrioma. I first saw my gyno at the end of February because of
pelvic pain on my left side, back pain (I have a tilted uterus), and
intense periods (like you said, "worse than labor"...even having BMs
is excruciating). I had a vaginal ultrasound, and at that time, the
cyst was 5cm. He prescribed BC pills (high estrogen), but they didn't
stop its growth (it's 7cm now). My doctor now wants to do a
laparoscopy and probably a laparotomy to remove the cyst and the
ovary. I wasn't sure if I wanted to research it much on the Internet,
because there is a lot of misinformation out there, but just decided
today that I had to. I hate feeling uninformed. As I suspected I
would, since starting the research, I have more questions than ever!
From what I'm reading, these things can be malignant (although
unlikely in our age group) and a CA-125 blood test can help determine
if it is. Also, some doctors are able to remove the cyst and ovary
through incisions much smaller than the one my doctor is talking about
using with the laparotomy. Like you, I'm relatively young at 34.
My doctor asked me if I wanted him to remove my uterus, too. Even
though I've had three children and my baby-making days are over, I am
so torn about having the uterus gone. My mother had hers removed at my
age (same problem), and had "intimacy problems" ever since. I'm
sorry, I know I don't have any helpful advice for you right now. I
have more questions than answers! It sounds like we're in the same
boat. I just wanted to reply to your message to let you know you're
not alone. On your last post, you said you had been bleeding for
nine days. I started bleeding 1 week into my last pill pack, and bled
for 5 days. I'm still spotting, but my doctor never mentioin that the
cyst could be leaking. Let me know how your last ultrasound went.

I am really young to, I am 29, and had a hysterectomy 2 years ago, and for about three months I felt wonderful. I have had unbearable pain. I have gone to the hospital for months and last night they finely gave ultarsound and said I had chocolate cyst, the pain has gotten so bad I wonder if ruputred and does anyone know if that is dangerous?
