Scoliosis occurs relatively frequently in the general population, and its frequency depends upon the magnitude of the curve being described. Scoliosis of greater than 25 degrees has been reported in about 1.5/1000 persons in the United States. Most curves can be treated nonoperatively if they are detected before they become too severe. However, 60 % of curvatures in rapidly growing prepubertal children will progress. Therefore, scoliosis screening is done in schools across America and several other countries. This screening is probably not necessary until the fifth grade. Beyond that point, boys and girls should be examined every 6 - 9 months. Generally, curvatures less than 30 degrees will not progress after the child is skeletally mature. Once this has been established, scoliosis screening and monitoring can usually be stopped. However, with greater curvatures, the curvature may progress at about 1 degree per year in adults. I believe your friend might have one major condition, which I believe is scoliosis, which leads to the other problems such as kyphosis and lordosis. My word of advice should not be taken as final for I have not seen your friend personally and hence I do not know clinically how he should be managed. Hence here is the typical advice base on the available physiotherapy machines in Malaysia. Singapore may have more advanced machines, so whatever I am about to discuss may be obsolete. Exercises or more often known as physiotherapy has their own unit under the guidance of a physiotherapist (they are not doctors, but are trained specifically in this field of allied medical health). It would be prudent to refer your information to them rather than to a junior house officer like me. However, I believe your friend who is having this condition must have worn braces throughout his illness to straighten up his spine alignment and that now he is trying to get back to his full range of movement. All physiotherapy do is to maximize their potential movements back to near-normal or best if, normal... The spinal exercises is divided into three parts. CERVICAL --> Neck exercises including flexing, extending, lateral flexion and lateral rotation... under the guidance of a physiotherapist, he will help your friend in the necessary exercise to get back to his fullest potential... THORACIC --> There is hardly any movements normally, required of this vertebraes, but it's importance in connecting the ribs to the front helps in respiration and breathing... hence there will be specific exercises which helps you to strengthen that column of spine, mostly there will be stretching exercises or ball exercises (where you sit on a ball and maintain your balance) LUMBAR --> This is the lowermost portion of your spine, and the exercises involve includes bending downwards touching your toes, bending upwards so that you see the sky, laterally (sideways) moving from side to side... The one that I know of where they use a soft and large pad (where soldiers after climbing a high wall, have to fall down onto a soft platform) where the patient will stand on it and maintain balance. Unlike that of the military's, this platform is specifically built in a slope... The patient will start at the beginning of the slope and then slowly maintain his balance and go higher and higher... if his pelvic muscles (supported by lumbar spine) is good, he will be able to reach the highest point. This exercise helps him to balance and make full use of his lumbar spine... Stretching is also useful for lumbar spine... The best method will still to be his own effort to maintain a straight posture whenever he sits, stands, moves around, and prior to being involve in sports, he should undergo several sessions of physiotherapy to encourage his muscles to build up and his joints to stabilize.
